A. Immunizations: Print and have your doctor complete the second page. Students must sign the last page. When complete, please fax to the Office of Admissions at 877-349-0231.
B. FERPA: Complete the “Authorization for Access to Student Records.” The authorization for access will allow The King’s College to disclose academic, billing, and student life information to a student’s parents. Students have the right to revoke this permission at any time.
C. Emergency Contact Information: This form must be filled out before arriving on campus. In the event of an emergency, our automated system will use this information to contact students and parents.
D. Disability Identification (For Eligible Students): The King’s College seeks to assure equal opportunity for access to and participation in all college courses, examinations and services by providing reasonable accommodations to individuals with physical, mental or learning disabilities recognized under the Americans with Disabilities Act of 1990 (ADA). In order to receive accommodations, eligible students must complete the Disability Identification form, as well as any other required documentation. Depending on the nature of the disability, a doctor’s report and/or other documents may be required. Please contact the Disability Officer, Sue Ho, at firstname.lastname@example.org for more information.
E. Verify Your Health Insurance: Proof of health insurance is required only for international students and student athletes. However, if you are covered by an HMO, PPO, or similar plan, we encourage you to determine the extent of coverage available, specifically in New York State.
If you do not have health insurance, we recommend students consider options available to them on the New York Health Exchange. International students may refer to independent providers such as International Student Insurance or Compass Benefits Group.
Step 6: Class Schedules
When your Admissions file is complete, the Office of the Registrar will compile your schedule.
You will be contacted with your schedule as soon as two months before the first day of class. This applies for both freshmen and transfer students. To learn more about the process view this page in the portal.
